We will ask you to confirm that the young person has given their consent, and we need their contact details so that we can get in touch with them directly.
Please note that we are a small charity and making a referral is no guarantee of receiving support.
Life Chance Trust will prioritise referrals based on our own needs assessment process and available resources at the time. If we feel we’re not the right source of help, or we need to place you on a waiting list, we will suggest other relevant sources of support.
