17 year old female with complex needs and family life. Animal obsessed, creative and kind.

Background
A was referred from her secondary school in year 11. A had not attended school since year 8 and school were worried about who would support her after she was off roll. A had experienced a lot of trauma in her childhood, and had lost her brother to suicide a couple of years previous. This had a massive affect on her and her Mums mental health. They rarely left the house, even though they both hated living there because of all the memories of their brother/son.

A was Life Chances Trust’s first referral.


I wish I’d had Chloe much sooner. I always want to see her and haven’t missed a session with her yet. We get on really well and she’s already helped me so much. She always supports me and is there for me when I need her. I wish I never had to be without her”.


Mentoring
A met with Chloe and a support worker from school on their initial meet. A and Chloe very quickly built a great rapport. They have kept in touch since she was initially referred in April 2023 and has re-referred herself. A see’s Chloe as her emotionally available adult and will reach out for support. In their time together, Chloe has supported A with housing, meetings, benefits, public transport, ID, CAMHs, sleep patterns, bereavement counselling, food and nutrition, being a young carer, referrals, completing forms, as well as help her build her resilience, self- confidence and ability to interact with others. They have also spent sessions visiting and interacting with animals, as this is something A finds therapeutic.

A and her Mums relationship is now in a better place, and A is going out a lot more, interacting with friends and family. She hopes to go and visit her other brother in north England.

She is also starting with CAMHs and they are going to support with moving into adult mental health.

“My daughter A has had various mentors and support workers over the last few years but has never engaged well. As soon as she met her new mentor Chloe, she connected with her. She really enjoys spending time with her and has lots of fun as well as different activities. As a parent with bad mental health, I really appreciate this service and Chloe. My daughter can do things that I’m unable to do with her at present time.

Chloe has also helped with a lot of practical things too such as housing, careers advice and bereavement counselling. Nothing is too much trouble and always helpful. Definitely the best support worker we have had and we wouldn’t change anything.”

Mum
